NOMINATION MATERIALS
ELIGIBILITY AND GUIDELINES:
To be eligible to receive the Kanter Prize, nominees must have first and foremost put into effect or developed an operational plan to significantly enhance the delivery of health care in the United States by minimizing disparities.
Nomination possibilities are unlimited and we encourage submission of all notable achievements that address the general definitions of enhancing health care delivery and disparity. Disparities include any inequality in health care delivery due to ethnicity, socioeconomic status, beliefs, race, religion, language, prejudice, bias, geographic constraints, and other factors.
Additionally, all nominees must be:
- A licensed Doctor of Medicine – MD or DO
- A United States citizen
- Currently practicing in the United States
- In good standing with his/her state of licensure
Individuals with any current or past affiliation with the Health Legacy Partnership (HELP), The Joseph H. Kanter Family Foundation, Agency for Healthcare Research and Quality (AHRQ), and/or Kanter Prize Selection Committee panelists are not eligible to participate.
- Only state, territory, military and District of Columbia medical associations/ societies may submit nominees.
- Individuals and organizations may recommend physicians to medical associations/societies directly but only nominees submitted by the associations/societies will be considered eligible. Contact information for each medical association/society can be found here.
- Medical associations/societies may nominate only one (1) physician each.
- Joint nominations are welcome (i.e. a single physician may be nominated to represent two or more medical associations/societies).
- Nominees must have developed a method or plan that has successfully enhanced health care delivery by minimizing disparities.
- Nominees are not required to be members of the nominating MD/DO association/society. However, the nomination is only valid if submitted through the aforementioned medical associations/societies.
- Nominators must be well acquainted with the candidate and/or his professional work to be able to attest to his/her qualifications and achievements.
- Nominators must be unrelated to the nominee by birth or marriage.
- The Kanter Prize committee encourages nominators to collaborate with their nominees to present a strong, well-informed nomination submission.
Only complete nomination packets received by February 6, 2009 will be considered. Packets must include:
- A signed nomination form including signatures from an authorized director or lay leader of the nominating association/society AND the nominee.
- A letter of nomination describing why the physician deserves consideration
- Please note the Kanter Prize is not solely awarded for humanitarian efforts. Nominees must have developed a method or plan to enhance the delivery of health care in the United States by diminishing disparities.
- A curriculum vitae of the nominee

Optional items include:
- Letter(s) of reference – maximum of three (3) letters
- Any supplemental information i.e. Biographical information, news stories, brief writings, reports, etc.
Additional information may be requested from nominees including, but not limited to field research committee and support documents of all nominees. Finalists should be prepared to participate in a phone/video conference and/or in-person interviews.

Please note that all submitted materials become the property of the Health Legacy Partnership (HELP) and the Joseph H. Kanter Family Foundation and may be copied, reprinted or published as part of the award program or other public communication. Copies rather than originals should be provided. No materials will be returned.
